Our program offers compassionate, hands-on support to help senior residents maintain their independence, access essential resources, and navigate age-related challenges with dignity.

We serve as the primary hub for senior support in Rolling Meadows, connecting residents to local programs, benefits, and assistance that promote safety, stability, and quality of life.

🔹 Referral Follow-Up

When other City departments identify seniors who may need additional help, our team provides timely follow-up to ensure every resident receives the support and services they require.

🔹 Case Management for At-Risk Seniors

We offer individualized case management to seniors facing complex needs. This may include:

Personalized care planning

Resource connection

Safety assessments

Occasional home visits to monitor well-being and provide support where it’s needed most
